Application Modernization Assessment Template
A practical framework for capturing application inventory, scoring business and technical factors, and building a prioritized modernization roadmap.
What an application modernization assessment should capture
A useful assessment begins with a consistent inventory. For each application, record the business owner, purpose, criticality, technologies, integrations, dependencies, hosting model, operating costs, lifecycle status, and known risks. Document evidence sources and mark unknowns explicitly rather than filling gaps with assumptions.
Business value
Measure process importance, strategic alignment, user reach, and the consequences of service disruption.
Technical health
Assess maintainability, support status, architectural constraints, performance, and integration complexity.
Risk and readiness
Evaluate security and operational risks alongside cloud suitability, data accessibility, and AI/automation readiness.
Suggested assessment workflow
- Discover: Build the inventory and assign accountable owners.
- Validate: Collect technical, financial and business evidence; flag missing data.
- Score: Apply documented criteria and weights consistently across the portfolio.
- Decide: Propose retain, retire, rehost, replatform, refactor or replace dispositions.
- Sequence: Consider dependencies, risk, capacity and business timing when building modernization waves.
- Review: Present findings to stakeholders and revise assumptions before execution.
Example inventory fields
Application ID · Business owner · Business capability · Criticality · Technology stack · Hosting · Integrations · Support status · Annual run cost · Technical health · Risk · Cloud readiness · AI readiness · Proposed disposition · Confidence level
A template provides structure, not an automatic architecture decision. Scoring should be calibrated to your organization and checked against real evidence.
Common assessment mistakes
- Using age alone as a proxy for modernization priority.
- Scoring applications without business-owner input.
- Ignoring shared dependencies and data migration constraints.
- Presenting a weighted score as a guaranteed return on investment.
